An Experimental Study of the Effects of Enflurane Anesthesia on the Kidney Function in Alloxan-induced Diabetic Rabbits

Abstract
This study was done to investigate the effects of enflurane anesthesia on the metabolism of enflurane and kidney function of alloxan induced diabetic rabbits. Forty rabbits were divided into two groups. Saline (2ml/kg in saline group, N = 20) or alloxan (100mg/kg in diabetes group, N = 20) was injected through the ear vein. And one week later, enflurane 1.5 vol% was administered via the endotracheal tube with O_(2) 2 L/min and N_(2)O 2 L/min for 2 hours in 15 rabbits of each groups, respectively.

The metabolism of enflurane was measured by blood and urine fluoride concentrations and kidney function was measured by urine volume, blood sodium, potassium and chloride concentrations, urine sodium, potassium and chloride excretion rates, fractional excretion of sodium, creatinine clearance and free water clearance. Blood and urine sampling was done before anesthesia, 0.5 MAC hour, 1 MAC hour, 1.5 MAC hour, 2 MAC hour, postanesthesia 1 hour and postanesthesia 1 day.

Nephrectomy was done to get the tissue sample for light microscopic examination in each groups, before and after anesthesia in 5 rabbits.

The results are as follows;

1. The urine fluoride concentrations were higher in diabetes group than control group and the urine fluoride concentration of 2 MAC hour and postanesthesia 1 hour in diabetes group were higher than preanesthesia value.
2. Urine volume was higher in diabetes group than control group and urine volume of both groups was decreased during anesthesia.
3. Blood sodium, potassium and chloride concentrations were lower in diabetes group and urine sodium, potassium and chloride excretion rate were higher in diabetes group than control group.
4. Creatine clearance was higher in diabetes group than control group and creatinine clearance of diabetes group was decreased during anesthesia and postanesthesia periods.
5. Fractional excretion of sodium was decreased during anesthesia in both groups but the value of 2 MAC hour and postanesthesia 1 hour were higher in diabetes group than control group.
6. Free water clearance was more negative in diabetes group than control group but free water clearance of diabetes group was increased to positive flection in postanesthesia periods.
7. Hyperplasia of mesangial matrix was observed in few samples of diabetic kidney, but there was no histologic difference between preanesthesia and postanesthesia groups.

The above results suggests that enflurane anesthesia on alloxan induced diabetic rabbit can produce more defluorination and more severe renal deterioration.
